A Clinical Review of the 1018 Emergent Explo-Lapa Cases in the Medium-Sized(200-250 bed) General Hospital


Abstract
I have tried to analyze the clinical findings about the total 1018 emergent explo-lapa cases which had been performed during four years from March 1989 to February 1993 in the Dept. of G.S. of Namseoul Hospital and Public Corporation Inchon
Medical
Center, which are the medium-sized (200~250 bed) general hospitals.
@ES The results are summarized as follows:
@EN 1) The total 1018 emergent explo-lapa cases were composed of 830 cases (82%, most common) of acute appendicitis, 99 cases(9.7%) of panperitonitis, 31 cases (3.1%) of hemoperitoneum (more than 2000 cc, without panperitonitis), 31 cases (3.1%)
of
intestinal obstruction, 6 cases(0.6%) of UGI bleeding, 6 cases (0.6%) of GB empyema, 5 cases (0.5%) of wound evisceration, 4 cases (0.4%) of intra-abdominal abscess, 3 cases (0.3%) of incarcerated inguinal hernia, 2 cases(0.2%) of intussusception
and a
cae (0.1%) of pancreatic pseudocyst.
2) Of the 830 emergent explo-lapa cases which had been performed due to acute appendicitis, there were 757 cases (more than 90%) of simple appendicitis, 35 cases of perforated appendicitis and 38 cases of periappendiceal abscess.
3) Total 99 explo-lapa cases of panperitonitis contained 59 cases (60%) of non-traumatic P. and 40 cases(405) of traumatic P. Of the 59 cases of non-traumatic P., there were 41 cases (69%, most common) of duodenal ulcer perforation, 6 cases of
gastric
ulcer perforation and 3 cases of colon cancer perforation etc. Of the 40 cases of traumatic P., there were 23 cases of small bowel rupture, 6 cases of colon rupture, 4 cases of stomach rupture and 3 cases of duodenal rupture etc.
4) Total 32 cases of hemoperitoneum contained 27 cases (84%) of traumatic H. and 5 cases (16%) of spontaneous H. Of the 27 traumatic H. there were 13 cases of splenic rupture, 6 cases of mesenteric tearing and 4 cases of liver laceration etc.
5) The 31 explo-lapa cases of intestinal obstruction were composed of 13 cases (43%, most common) of adhesive ileus, 6 cases of duodenal obstruction (5 cases due to duodenal ulcer obstruction, 1 case due to CBD cancer invasion), 5 cases of colon
cancer
obstruction, 2 cases of sigmoid volvulus and 2 cases of small bowel ileus etc.
6) Total 68 cases of emergent explo-lapa due to trauma contained 56 cases (82%) by blunt trauma and 12 cases (18%) by penetrating trauma. Of the 56 emergent explo-lapa cases by blunt trauma, there were 20 cases (36%, most common) of small bowel
rupture, 13 cases (23%, 2nd common) of splenic rupture, 6 cases of mesenteric tearing 5 cases of colon perforation, 4 cases of duodenal rupture, 3 cases of liver laceration and 2 cases of portal vein branch tearing.
